What to Include in Your Referral

Patient Details

Full name, date of birth, and contact details
Medicare number and DVA number (if applicable)
Private health insurance details (if applicable)
Next of kin / support person details

Clinical Information

Referring doctor's name, provider number, and practice address
Reason for referral and primary clinical concern
Diagnosis or suspected diagnosis (with histology/pathology if available)
All relevant pathology results and tissue biopsy reports
Copies of recent imaging (CT, PET, MRI, ultrasound, X-ray)
Previous oncology or specialist letters
Current medications and known allergies
Relevant co-morbidities and functional status

Urgent Referrals

For urgent referrals, please telephone the rooms directly
Urgent appointments are usually available same-day or next business day

Cover & Continuity of Care

Dr Ade Freeman (MBChB, FRACP) works with COGMO in association with Dr Kiberu and provides specialist medical oncology cover
All COGMO patients have continued access to a consultant medical oncologist at all times
Dr Freeman also consults at St John of God Murdoch Hospital, bringing additional expertise to the team

After the Consultation

A detailed specialist letter will be sent to your practice following the consultation
Where systemic therapy is initiated, regular GP liaison letters will be provided
Please ensure patient contact details are current for appointment reminders
